Below are links to various patent forms for use with USPTO applications with a post-2012 filing date.  You may have to open these forms outside of your web browser to view their contents.  All forms are for online EFS submission.

In patent law, the inventor(s) is(are) responsible for the subject matter of the claims.  Typically, the assignee is a company that the inventor works for or contracted with under an IP assignment clause, or a company to which patent rights were sold.
